Comments:
An unannounced monitoring inspection was conducted on June 9 2023 between 9:15am and 10:15am. There were no children in care at the time of inspection. A review of children's records, staffing records, and evacuation drills was conducted. A discussion was conducted with provider about the upcoming application for a special use permit to increase capacity. The provider was informed that with 12 children a need for an assistant may be needed. Evidence: Child #1 information for parents document did not contain specific information on hours, fees, or termination policy. The medication Administration document was not completed. The liability insurance declaration was not filled out. Child #2 information for parents document was not completed, Mediation Administration document was completed. Liability declaration was not completed.
