Comments:
A mandated monitoring inspection was conducted today from 10:30 to 11:45 am. There were 10 children present (20 points) with 2 caregivers. Children were observed writing in their workbooks, building with magnetic blocks, washing hands, coloring, and having circle time. Reviewed 2 children's files, 2 caregivers files, 3 resident's files, 1 pet file, emergency evacuation plan and log, first aid kit, attendance, menus and physical plant. Based on record review, children's records did not contain required information. Evidence: Child #2 record was missing 1 parent's work phone number; 1 emergency contact last name, address and phone number; and medical insurance policy number.
