Technical Assistance:
Discussed the following with the provider:
1. Hand washing procedures for young infants; 2. CPSC guidelines for cribs - written information emailed; 3. Requirement for a carbon monoxide detector - written information provided.
Comments:
An unannounced renewal inspection was conducted on 11/28/2023 from 10:35 am - 12:30 pm. There were four children in care with the provider. The ages of the children counted for a total of 14 points.
Children were observed during morning program time which included snack and free play. Two diaper changes reviewed and one infant was observed being bottle fed by the provider.
Records were reviewed for the four children in care and the provider. The entire home and backyard play area was additionally reviewed.
Three violations were observed and are listed on the violation notice.
Based on record review, the provider failed to ensure that children's records contain a the results of the health examination within 30 days of enrollment.
Evidence: 1. Child 1, enrollment date 05/20/2022, does not have documentation of a health exam. 2. Child 2, enrollment date 05/10/2022, does not have documentation of a health exam. 3. Child 3, enrollment date 01/08/2023, does not have documentation of a health exam. 4. The provider confirmed that documentation of a physical exam was not on file for these 3 children.
Plan of Correction:
The provider stated that each parent will be asked to provide a current physical within the week but no later than 30 days.

Description:
Based on observation and interview, the provider failed to meet all of the criteria on the health and safety checklist.
Evidence: 1. The phone number for 911 or emergency telephone numbers were not posted in clear view. a. The provider confirmed that emergency telephone numbers / 911 were not posted. 2. There was no written documentation on file to verify if one crib in use meets current CPSC guidelines. a. The manufacturer label was torn off therefore the manufacture date could not be determined and the provider stated she did not have any written documents for the crib.
Plan of Correction:
The provider stated that the number for 911 will be posted today.
The provider stated she will contact the manufacturer of the crib to determine if the crib meets current CPSC guidelines. If the crib does not, a new crib will be obtained.
