Comments:
An unannounced renewal inspection was conducted on 10/09/2019 from 11:20 am - 12:35 pm. There were three children in care with the provider. The ages of the children counted for a total of ten points. Children were observed during morning program time. One infant was observed napping in the portable crib. Records were reviewed for three children in care and the provider. The entire home and backyard play area was additionally reviewed. Two violations were observed and are listed on the violation notice.
Based on record review and interview, the provider failed to ensure compliance with all of the required elements of the health and safety criteria and checklist.
Evidence: 1. There was no documented physical exam on file for child 1 who was in care during the inspection. a. Child 1 has an enrollment date of 08/12/2019. b. A physical exam is required within 30 days of enrollment. 2. The rabies vaccination for the two family cats expired on 04/07/2019.
Plan of Correction:
1. The parent of child 1 was contacted during the inspection to request a physical. Child 1 has an well baby appointment next week. Documentation of a physical will be obtained during this appointment. 2. The Humane Society will be contacted in order to schedule an appointment for an updated rabies vaccination for the two family cats. Both cats are indoor cats.
