Technical Assistance:
Discussed with provider: Updating the medication policy, no medications can be administered without MAT certification, even OTC medications. (No medications present) Updating the check-in/check-out policy for clarification. Memo sent regarding provider portal on the DOE website.
Comments:
An in person renewal study was conducted today. There were 2 children (8points) with 1 caregiver. 1 caregiver and 2 household records were reviewed. All were complete. 2 child records were reviewed. See Violation Notice for non-compliance. First Aid supplies, emergency evacuation drills, emergency supplies, fridge/freezer temperatures, 3 cribs, age appropriate equipment were all in compliance. The inspection will be typed off site and emailed for signatures. CPS searches for all adults will expire in July. Please submit new requests as soon as possible to prevent a delay in receiving the new results. Based on a review of record, one child record does not contain written acknowledgment of all required policies and procedures prior to enrollment. Evidence: 1 of 2 records reviewed did not contain parent's signature and date on the policies and procedures document. (Child 2)

I will have the parent sign the form at pick up.
