Discussed with provider: One caregiver TB result will expire in May. Your renewal application will come via email 90 days in advance of your expiration date. It should be completed and submitted through the online portal.
Comments:
An unannounced monitoring inspection was conducted today. There were 5 children present (13 points) with 2 caregivers. 2 child and 2 caregiver records were reviewed. See Violation Notice for non-compliance cited. Emergency evacuation drills, fridge temperature, diapering and bathroom supplies were all in compliance. Children were observed playing with a variety of age appropriate toys.

Description:
Based on a review of records, one child record did not contain written acknowledgment of all required policies and procedures. Evidence: Child #1's record did not contain documentation of the decision to administer medication. The document is 2 pages. Page 2 was missing with the parent's signature.

I will have the parent sign page 2 and place it in the file.
