Comments:
An unannounced renewal inspection was conducted today from 10:55am-1:15pm. There were 73 children (infants-5yrs.) directly supervised by 11 staff. The physical plant, staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating lunch. Diapering and hand-washing procedures were also observed. There was an ample supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). If a plan of correction is not received within the required timeframe, it will not appear on the public website.

Violations:
Standard #: 8VAC20-780-50-A
Description: Based on observation, child information was not kept confidential.
Evidence:
The allergy list with identifying student information and action plan was posted taped to a cabinet in the classroom without a cover sheet.

Plan of Correction: All allergy lists are placed in each classroom with a cover page for privacy. Ms. VK updates allergy lists monthly and will post in the classroom with a cover page. Ms. Dani will check classrooms to ensure cover pages stay in place.

Standard #: 8VAC20-780-60-A
Description: Based upon record review, required child information was incomplete.
Evidence:
Child A's record is missing parent work contact information.
Child B's record is missing parent work contact information.
Child c's record is missing parent work contact information.

Plan of Correction: All child files will contain the new Family Emergency Card form complete with both parents work place information and updated contact information. Ms. VK will collect from families and Ms. Dani will review for completion.
Child A, B, and C records were all updated on 4-23-24.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment were not maintained in clean and safe condition.

Evidence:
Infant classroom - There was peeling paint in window area by the kitchen area.
Playground- There were square holes next to fence posts that had been replaced that were located in the path of the sidewalk that pose a tripping hazard. There were metal wires that are exposed on the utility box on the playground wall.

Plan of Correction: Work orders have been placed under "urgent" for repair. Ms. Dani and Ms. VK will clear the work orders out of the system once they have been completed by facilities.

Description: Based on observation, cleaning materials were not kept in a locked place to prevent access by children.
Evidence:
In classroom 3A cleaning products were observed in in unlocked cabinet above the sink.

Plan of Correction: Cabinets containing cleaning products will be kept locked at all times, in all classrooms. Daily classroom inspections will be conducted by Ms. Dani and Ms. VK to ensure 100% compliance.

Description: Based on observation, cleaning materials were not kept in a locked place to prevent access by children.
Evidence:
In classroom 3A cleaning products were observed in in unlocked cabinet above the sink.

Plan of Correction: Cabinets containing cleaning products will be kept locked at all times, in all classrooms. Daily classroom inspections will be conducted by Ms. Dani and Ms. VK to ensure 100% compliance.

Description: Based on observation, children's hands were not washed after contact with urine.
Evidence:
A child in the preschool classroom had a potty accident on the playground while doing head counts. The child came into the classroom and waited while walking around the classroom for their turn to use the bathroom and change.

Plan of Correction: Children who are soiled/ wet will go directly into the bathroom to be cleaned up and changed. They will immediately wash their hands. Classroom teachers will be responsible for ensuring health and safety measures are maintained when bathroom accidents occur. Ms. Dani and Ms. VK will observe to ensure compliance.
