Technical Assistance:
Discussed the following with the caregivers: - Updating children's records - Acceptable forms of attendance - Crib requirements - Annual training required
Comments:
An unannounced monitoring inspection was completed today. The physical plant, 3 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, and supplies were inspected today between 10:40-11:30am. There were 9 children, totaling 28 points, under the supervision of 2 caregivers. Today, the children were observed enjoying free play, having diapers changed, cleaning up for and eating lunch. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served fish sticks, pasta, chicken noodle soup, mixed vegetables and milk. At this time, the Provider is not certified in MAT (medication administration training) and there are no children with any medication needs. Based on file review, 2 out if 5 children's records have not been updated annually. Evidence 1. Child #4's record was last updated on 11/1/18. 2. Child #5's record was last updated on 1/4/19.

Description:
Based on file review, 2 caregivers do not have a TB test/screening completed within the past two years. Evidence 1. Caregivers #1 and #2 have TB tests which were completed on 10/22/17.
