Description:
Based on a review of caregiver and household member's records, the provider failed to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence: 1) Caregivers #1 and 2's records do not have documentation of a current tuberculosis screening form. Caregiver #1 and #2's previous screenings were 11/2/2017. 2) Household member #1's record does not have documentation of a current tuberculosis screening form. Household member #1's previous screening was 11/2/17.
Plan of Correction:
We will complete TB Screening on Monday, November 12, 2019
