Comments:
An unannounced monitoring inspection was conducted today from 11:50am-12:35pm with the provider on site. There were 11 children present with 3 caregivers. A sample of 2 children's files and 7 caregiver/household files were reviewed. The children were engaged in the following: playing with toys. reading books with the caregivers and sleeping. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and caregivers were positive. The physical space, attendance records, drill record, first aid kit, radio, flashlight, and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Repeat Violation Based on review of four household members records, the provider did not obtain a completed sworn disclosure statement for each household member within 30 days of their becoming 18 years old. Evidence: 1. Household Member #3 did not have a sworn disclosure statement on file and has been 18 years old for more than 30 days.

Description:
Based on interview, one caregiver did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. Evidence: 1. Caregiver #1 completed 14 hours of training from 2/2022 to 2/2023.

I will complete the training hours for 2/2023-2/2024.
