Comments:
A subsidy health and safety (SHSI) inspection was completed on 1/30/2024 from1:00 pm - 4:35 pm. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined violations with applicable standards or law which are identified on the violation notice.

Violations:
Standard #: 8VAC20-790-540-B
Description: Based on a review of records and interview, the vendor did nto always ensure that each child's record contained all of the required information.
Evidence:
Five children's records were reviewed.
The record for child #1 who enrolled in the program 9/6/2022, did not have the address listed for the designated emergency contacts to be called in the event of an emergency of the child's parent cannot be reached.

Plan of Correction: The parent will be asked to provide the emergency contact's addresses at the time of pick up this afternoon and the address will be documented in child #1's record by the next day.

Standard #: 8VAC20-790-600-F
Description: Based on a review of records and interview, staff who work directly with children do not always obtain in addition to preservice and orientation training at least 16 hours of childcare related training and staff development activities, to include the department's health and safety update course.
Evidence:
Eight staff records were reviewed.
The records for staff #1 hired 7/25/20225, contained documentation of only 6 of the 16 required hours of applicable training for the 2022 / 2023 period.

The record for staff #4, hired 12/4/2020 contained documentation of only 3 of the 16 required hours of applicable training for the 2022 / 2023 period.

The record for staff #8, hired 3/9/2022 contained documentation of only 11of the 16 required hours of applicable training for the 2022 / 2023 period.

Plan of Correction: Staff will be reminded that they are required to complete 16 hours of training annually and have already begun obtaining and documenting their training hours for the current period. Admin will review the training hour accumulation well before the 12 month period ends to ensure each staff completes a minimum of 16 training hours, as required.
