Comments:
An unannounced monitoring inspection was conducted on 1/12/23 from 9:30am to 12:10pm with the Director and Assistant Director. There were 91 children in care, ranging in age from five-months to five-years-old, supervised by 17 staff. The children were observed having structured play in centers, having diapers changed, participating in circle time and singing songs, playing a ?following directions? games, and playing on the playground. Six child records and four staff records were reviewed. The Center has 22 staff with current certification in CPR and First Aid, and 19 staff trained in Daily Health Observation Training. Nine medications and authorization forms were reviewed and the center has eight staff current in Medication Administration Training (MAT). The attendance, menu, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 6/16/22 and the most recent Health Inspection on file was dated 7/21/22. Lunch served was pesto pasta with peas and chickpeas, green beans, mandarin oranges, and milk. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 1/19/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-780-350-B-1
Description: Based on observation, interview, and record review, the center did not ensure that the required ratio of staff to children were met. Evidence: On 1/12/23 there were nine children observed in the Toddler classroom with two staff. The youngest child in the class was 15-months-old and required a ratio of 1 staff: 4 children.

Plan of Correction: Had each parent for younger age child sign off on a ratio letter that they are okay with the higher ratio. Also attached assessments from the child's teachers they are coming from and going to, to show readiness.

Description: Based on observation, interview, and record review, the center did not ensure that the required ratio of staff to children were met.
