8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes
Comments:
A subsidy health and safety (SHSI) inspection was initiated and concluded on 01/18/2024. There were 13 children present (including 2 resident children) with 3 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review of records one child record did not contain all required documentation. Evidence: Child #5 had a documented food allergy. There is no written care plan available for review including physician's instructions.

I will ask the parent to provide the plan.

Description:
Repeat Violation Based on a review of caregiver records, the required annual updated training for caregivers was not completed. Evidence: *2 of 3 caregiver's certificates were dated 2/15/22. (Caregiver A, B) *1 caregiver record had no certificate of completion available for review. (Date of Hire: 7/25/22)
Plan of Correction:
The class will be taken as soon as possible.
