8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes
Comments:
A subsidy health and safety (SHSI) inspection was initiated and concluded on June 4, 2024. There were 14 children present (including 3 resident children) with 3 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Repeat Violation Based on caregiver's statement and a review of records, caregivers had not completed the required annual health and safety update course. Evidence: Caregiver A - last course taken was dated 2/15/22 Caregiver B - last course taken was dated 2/15/22 Caregiver C - has no documentation of taking the course.
Plan of Correction:
Both of us will complete the course.
