Comments:
A Subsidy Health and Safety inspection was conducted on January 14, 2020 to review supplemental health and safety requirements. This on site visit was from 10:05 a.m. until 1:35 p.m. There were twenty two children in the care of three staff and one volunteer in three classrooms. The administrator was also available to assist, if needed. Five children?s and four staff records were reviewed and updated. No medication is being administered at this time. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on review of staff records, the program failed to ensure that all staff complete the Virginia Preservice Training within 90 days of subsidy vendor approval.
Evidence: 1. The records for Staff #2, #3, and #4 did not contain certificates for completion of the required training. 2. Program became an approved subsidy provider on September 27, 2019. 3. Administrator confirmed that she was the only staff member that had completed the training.
Plan of Correction:
Staff will all take Required training for subsidy by Feb 1
