Comments:
An unannounced monitoring inspection was conducted on 1/23/2023 from 10:30 a.m. 11:30 There were 8 children present, ranging in ages from 10 months to 4 years old, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of records and interview, the provider failed to obtain documentation that all children had be adequately vaccinated before the first day of attendance. Evidence: 1. Child #1?s first date of attendance was 8/15/2022. The vaccinated record was dated 8/24/2022. 2. The provider confirmed that the vaccinated record was not received before the first day of attendance.
Plan of Correction:
When I do files for new families, I will have everything before they start.
