Please forward completed fingerprints to the Licensing Inspector at jacqueline.ware@dss.virginia.gov as soon as they are received.
Comments:
Licensing Inspector arrived at 10:00 and departed at 11:15 AM to conduct an initial health and safety inspection in conjunction with a voluntary registration monitoring. There are currently no children enrolled in this home. Records for the provider and other household members were reviewed.
Based on review of household records, it was determined that the vendor did not ensure that all staff submitted to fingerprinting as required.
Evidence: Household members #1, #2 and #3 did not have completed fingerprints on file.
Plan of Correction:
Provider was given instructions to obtain fingerprints. Provider will ensure that all persons in household obtain fingerprint checks.
