Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on March 16, 2021. The provider was contacted by telephone to initiate the inspection. There were four children present(12pts) and two staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review and interview, the provider has not annually updated the emergency contact information.
Evidence: 1. Child #1's record (First date of attendance-3/4/20) was not reviewed annually. The record was last reviewed on 01/28/20. 2. Child #2's record (First date of attendance-7/22/19) was not reviewed annually. The record was last reviewed on 7/22/19.

The provider will review the record with parents and have them to sign the record to acknowledge the review.

Description:
Based on record review and interview, the family day home did not complete a written emergency preparedness plan.
Evidence: 1. There was no documentation of a written emergency preparedness plan for the family day home. The provider had a blank copy of a plan, but had not filled in the information. The provider stated that the plan was not completed.
Plan of Correction:
The provider will complete a written emergency preparedness and response plan and send it to the licensing inspector.

22VAC40-111-830-A
Description:
Based on record review and interview, emergency evacuation procedures were not practiced monthly with all caregivers and children in care during all shifts that children are in care.
Evidence: 1. There was no documentation of emergency evacuation procedures practiced for the months of January and February. The provider confirmed that the monthly drills were not completed.
Plan of Correction:
The provider will perform a monthly fire drill for the month of March and make up for the missing fire drills by performing two additional drills in the upcoming months.

Description:
Based on observation and record review, the family day home did not keep a written record of children in attendance each day.
Evidence: 1. There was no documentation of the children's arrival on the attendance sheet for this date.
Plan of Correction:
The provider corrected this during the inspection.
