The inspector emailed the provider additional forms and information.
Comments:
A renewal inspection was conducted on 06/29/2022 between the hours of approximately 11:45 am and 2:00 pm. There were 5 children present, ranging in ages from 9 months to 5 years, with the caregiver. A family day provider that is voluntarily registered should not care for more than four children at any one moment so the amount of children under care was outside of the supervision guidelines.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. No medications were on site today. A complete walkthrough of the family day home was conducted. A total of 5 children?s records and 3 caregiver/household member records were reviewed.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observation, the provider did not ensure that the total number of children receiving care at any one time does not exceed four. Evidence: 1) On the date of inspection, five children were receiving care in the home.

I made schedule arrangements for not to exceed 4 kids at the same time.

Description:
Based on record review, children's records did not contain all required information. Evidence: 1) The record for Child #1 did not contain the results of a health examination and up-to-date immunization records of the child. 2) The record for Child #2 did not contain the name, address and telephone number of a second parent's place of employment and their work hours; the address and phone number of the child's physician. 3) The record for Child #3 did not contain the work hours for one of the parent's place of employment.

Description:
Based on observation, documentation review and interview with the provider, it was determined that some criteria on the health and safety checklist was not met. Evidence: 1) Evacuation drills were not practiced on the months of January 2022 and February 2022. 2) Although there was a flashlight in the home, it was not operable since the batteries had died. 3)The temperature in the home's refrigerator compartment was kept at more than 40 F. (50 degrees approximately) 4) An evacuation map was posted, however, it was incomplete since it did not indicate the route and point of exit from the home. 5) Hazards were observed at the outdoor play area used by children: a small garden trowel was accessible to children and a long nail was being used to latch the shed door. Both of these objects have pointy edges that could snag the skin of children.
Plan of Correction:
1) Practiced evacuation drills for Jan 22 and Feb 22. 2) Changed batteries for flashlight. 3) St fridge temperature to 40 4) Marked point of exit in the evacuation map. 5) Removed the garden trowel and replaced nail for a lock.
