Comments:
A unannounced complaint inspection was conducted with the owners and staff. After interviews with staff and a review of the video recording it was determined that the complaint was valid. Regarding a bathroom door being closed on a three year old child's finger. There was one violation cited during the inspection that was unrelated to the complaint. Based on review of Staff A's file it was determined the staff does not meet the education for Program leader qualification. Evidence: Staff A with a hire date of 3/4/19 Does not have documentation of a high school diploma or the equivalent.
Plan of Correction:
Request of proper documentation to complete the staff file. Until file is complete we will place another lead teacher in the classroom.

Description:
Based on staff interviews and review of the video recording it was determined that the staff did not ensure the care and protection of a three year old child in care. Evidence: Staff A In the Penguin room did not put the child safety lock on the bathroom door. Child A opened the door to use the bathroom and another child pushed the door closed on child A's finger. Breaking child A's finger.
Plan of Correction:
Retrain staff on how to use child locks on the doors.
