An investigation was initiated from 12:00 pm - 1:00 pm to investigate a self reported incident regarding supervision, and care, protection, and guidance. Interviews were conducted, and documentation was reviewed. The investigation regarding supervision, and care, protection, and guidance is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on interviews it was determined that the center failed to ensure the care and protection of children at all times. Evidence: On 08/23/2022 child A (DOB 04/01/2017) did not remain under the care and protection of staff for a time period of two minutes. Staff A was unaware that child A was still on the center's bus after the other children departed from the bus. Staff A stated that a head count was conducted outside of the bus however, it was not discovered that child A was not present with the group. A name to face check was not conducted. Staff A and the group of children entered the facility. Child A began banging on the door and trying to open the door when they were discovered by a staff member on the center playground.

Child A was discovered two minutes later by a staff member on the center playground.
