Please forward copies of the results for the Federal Finger Print Based background checks for provider #1 and household member #2 as soon as you receive them after they are conducted on May 23,2020.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 5/5/2020 and concluded on 5/5/2020. The provider was contacted by telephone to initiate the inspection. There were 2 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
Based on a review of records and interview, the provider did not ensure that all criteria on the Health and Safety Checklist are met. Evidence: 1. The provider did not have all required documentation requested for the 2 enrolled children on the date of the inspection. - Two children's records were reviewed. - The provider did not have a copy of the physical examination results obtained on child #2 before or within 30 days after enrollment. - The provider did not have Documentation of reviewing proof of child #1's identity and age. - The provider stated that she did not have this documentation available. 2. The provider did not have documentation of having practiced emergency evacuation drills monthly to the point of exit from the home. - The reviewed emergency drill log did not have documentation of an evacuation drill having been conducted during the month of August 2019. - When asked, the provider stated that she more than likley donducted the evacuation drill but forgot to document it.
Plan of Correction:
The provider stated that she would ask the parent of child #2 to provide a copy of child #2's last physical today. The Provider stated that she would assist the parent of child #1 with obtaining a copy of child #1's birth certificate today. The provider will ensure that evacuation drills are conducted and documented monthly beginning with this month.

Description:
Based on a review of the application and attachments as well as interview, the provider did not submit to fingerprinting described in subdivision B 2 of ? 19.2-392.0. Evidence: The provider did not submit results of a finger print based federal criminal history search for herself or household member #2. When this documentation was requested the provider stated that she thought the State Criminal Record Search met the requirement until 5 years had passed and that she had not requested these background checks.
Plan of Correction:
An Appointment was made to have the finger print based federal criminal history search conducted for herself and for household member #2. The appointment confirmations set for May 23, 2020 at 10:30 a.m. and 10:40 a.m. were emailed to the inspector on April 24, 2020.
