Discussed with church administrative staff: Health reports done annually, First aid & CPR must have an in-person component, Daily Health Observation course.
Comments:
An unannounced monitoring inspection was conducted today. Staff records were reviewed for specific items only. Fire inspection report, Health inspection report were obtained. The Statement of Intent was amended with the Clerk of Session signature.
LI was present today from 12:30PM - 1:40 PM.

Based on a review of records, staff records did not contain all required documentation regarding staff health reports. Evidence: 1 of 5 records reviewed was missing a completed staff health report (Staff #5). 2 of 5 records reviewed did not contain an annually updated staff health report. Staff #1's health report was dated 1/25/22. Staff #2's health report was dated 8/20/21.
Plan of Correction:
All staff will have new "Staff Health report forms completed as soon as possible.
