Comments:
An unannounced monitoring inspection was completed on 12/12/2019 from 9:00 A.M- 1:30 P.M. At the time of the inspection 101 children were in care with sixteen staff and three administrators present. Ten children's records, ten staff records, required posted information, evacuation drills, five injury reports, Welcome Handbook, health inspection, first aid kits and supplies,over-the-counter skin products and required documentation, and five emergency medications and required documentation were reviewed. Children were observed during infant naptime, infant feeding, story time, free choice activities, art activities, shape recognition activity, dancing and movement, handwashing, diapering, behavioral guidance, lunchtime, and naptime. The exceptions to compliance are noted on the violation notice.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of staff files, the center failed to ensure that staff had submitted documentation of a negative tuberculosis screening no later than 21 days after employment.

Evidence:
1. Staff #8's tuberculosis screening was dated 09/11/2019. Date of hire was 08/13/2019.
2. Staff verified the date of hire and the date on the tuberculosis screening for Staff #8.

Plan of Correction: TB screening will be collected prior to the first day of work.

Standard #: 22VAC40-185-520-A
Description: Based on a review of over the counter skin products, the center failed to ensure over-the counter skin products shall not be kept beyond the expiration date of the product.

Evidence:
1. In Classroom #3 there was a tube of Bordeaux Butt-paste that expired in 07/2018.
2. Staff verified the expiration date on the over the counter skin products.

Plan of Correction: Staff will be reminded to check all expiration dates on over the skin products when receiving them from the parents. Internal inspections will be conducted one time per month on all products, that they confirm that products are not expired.
