Comments:
An unannounced renewal inspection was conducted on 4/8/2024 from 12:50pm-1:40pm. There were 3 children present. The home, 1 staff record, 1 household member?s record, evacuation drills and emergency supplies were inspected. There was an adequate supply of books, toys, and materials for the children. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Based on record review the provider did not submit or obtain sworn statement or affirmation for an adult living in the home. Evidence: HHM#1 did not have a sworn statement on file.

Provider will obtain sworn statement from HHM.

Based on record review and interview, the provider did not obtain results of a search of the central registry for required persons. Evidence: The provider and HHM#1 had not submitted a search of the central registry.
