Comments:
An unannounced on-site monitoring inspection was conducted on 10/31/2023. There were 16 children, ages 5 years-12 years, and 3 staff members present during the inspection. The inspector observed the following: arrival free choice activities, and snack. A total of 5 children?s records and all required policies and procedures were reviewed. The inspector arrived at 2:30 PM and departed at 4:00 PM.

Based on record review, the center failed to ensure that the separate record for each enrolled child shall contain all the elements as required by the standards.
Evidence:
1. The record for Child #2 did not include the following information: first day of enrollment. 2. The record for Child # 3 did not include the following information: previous childcare experience. 3. The record for Child # 5 did not include the following information: physician?s Phone Number.
Plan of Correction:
Staff will have the parent to update the information that is missing and staff will monitor all record for completeness.

Based on observation, the center failed to ensure that all areas and equipment are safe, clean and operable condition.
Evidence:
1.A chair in the main child care room was missing the left arm rest and had exposed medal that was accessible to the children.
Plan of Correction:
The chair will be removed and the staff will monitor.
