Comments:
An unannounced on-site monitoring non mandated inspection was conducted on 11/20/2023. There were 18 children, ages 5 years-12 years, and 2 staff members present during the inspection. The inspector observed the following: during arrival, preparing for snack and handwashing A total of 2 children?s records and 1 staff record, were reviewed. The inspector discussed the following with staff: hire date vs start date in correlation with background dates. The inspector arrived at 2:30 PM and departed at 3:40 PM.

Based on review of eight children?s records, the center failed to ensure that all records were complete per the standard.
Evidence:
1.The record for Child # 2 had documentation of the first day of attendance of 11/13/2023 was missing the addresses for both of the two emergency contacts, and the physician?s contact phone number. 2. The record for Child # 3 was missing documentation of the father?s information, allergies and action to take and physician?s name and contact phone number.
