Comments:
A complaint investigation was initiated on September 26, 2019 with another local agency and ended on October 7, 2019 regarding the missed feeding schedule of a 4 month old. Interviews were conducted, and documentation was reviewed. There was a preponderance of evidence to support the allegation so the complaint was deemed valid. The complaint related violations are identified on the Violation Notice. Description: Based on staff interview and documentation, the center did not schedule snacks or meals so there is no more than three hours between each meal or snack.
Evidence:
Staff interviews and documentation showed Child#1 (4 months old) received 2 bottles while in care for 9 and a half hours. The child was present at the facility from approximately 7:45 am to 5:20 pm. Interviews with staff revealed the child was fed "in the morning and around noon". Staff stated the child did not get another bottle after the noon feeding.

Plan of Correction: Feeding schedules re-introduced and monitored

Description: Based on staff interviews and documentation, infants were not being fed in accordance with parental instructions.
Evidence:
Interviews with multiple Staff consistently revealed Child#1 (4 months) was "given a bottle in the morning and around noon" before being put down for a nap. Staff further stated Child#1 did not receive a bottle after waking from nap which was not in accordance with parental instructions.

Plan of Correction: Staff was re-trained in feeding procedures and following parental instructions
