An investigation was initiated on 12/10/19 in response to a self-reported incident related to lack of supervision of a child in care. This was received by the Licensing Inspector on 11/21/19. There were also follow-up interviews on 12/16/19 and 1/24/20. On 11/21/19, the director reported an incident where a child was left without direct supervision on 11/20/19 from approximately 4:48pm-5:15pm. See Violation Notice for more details. After interviews were conducted and video surveillance was reviewed, this reported incident was determined to be valid. Based on interview and observations, the center failed to ensure that all children under 10 years of age are always within actual sight and sound supervision of staff. Evidence 1. On 11/20/19 at approximately 4:48pm, child #1 went into a bathroom stall, which adjoined his classroom. At 5:09, staff #1 lined up the remaining 7 children in the classroom and left the class to transition to another classroom. During this time, child #1 was still in the bathroom by himself. At approximately 5:15pm, child #1's relative came to pick the child up. Once staff #1 and #2 realized child #1 was not with their group of children, they went to look for the child. Staff #2 found the child, who was still in the bathroom stall.
Plan of Correction:
On 11/21, the Director left the LI a voicemail and met with the parent the following morning to discuss the incident. Staff #1 went out of town on 11/22 and did not return to the center until 12/23 . Upon her return, the director gave staff #1 a written warning and reviewed procedures for supervising children (including conducting name-to-face).
