Comments:
An unannounced on-site non mandated inspection was conducted on 10/30/2023. There were 8 children, ages 4 years-8 years, and 2 staff members present during the inspection. The inspector observed the following: arrival, free choice activities. A total of 5 children?s records, were reviewed. The inspector arrived at 6 AM and departed at 7 AM.

Based on record review, the center failed to ensure that each report of immunization and physical examination shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.
