Comments:
An unannounced mandated inspection was conducted on January 29, 2024. The inspector was on site from approximately 9:30 am until 10:30 am. There were four children present with one caregiver supervising for a value of 7 points. Later, one additional child arrived for a total of nine points. Children were observed playing with toy trucks, play food, and toy dolls, playing dress up and sharing toys.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records, one caregiver record and one household member record were reviewed. The inspector interacted with the provider and children in care. The children were excited to meet the inspector and share what they were playing with.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on record review and interview, the provider did not ensure to obtain for each household member a current report of Tuberculosis (TB) in accordance with the requirements in 8VAC20-800-180, every two years from the date of the first screening or more frequently as recommended by the physician or the local health department.
Evidence:
1. The record for Household member #1 contained a tuberculosis result dated 8/14/21. 2. The provider acknowledged that the TB screen had not been obtained every two years. Plan of correction:
