Comments:
An unannounced monitoring inspection was conducted between the hours of 10:00 am and 11:30 am. Upon arrival there were 5 children in care, totaling 11 points with 1 caregiver. A sample of 3 provider/household and 3 children's records were reviewed. Children were observed playing with toys, drawing and reading books.
Based on a record review the licensee did not meet all requirements when maintaining children's records. Evidence: 1. Child #1 (enrolled 03/22/18) did not have documentation of having a physical examination or the phone number to the child's physician. 2. Child #2 (enrolled 09/27/19) did not have documentation of 1 of the 2 required emergency contact addresses.
Plan of Correction:
The following corrective action was provided by the licensee during the exit interview: These were both an oversight. The licensee will contact the children's parents today to obtain this information and document it in the record.
