Comments:
An unannounced monitoring inspection was conducted on 5/8/24 from 8:50am to 11:50am with the recently promoted Assistant Director and Director. There were 91 children in care, ranging in age from three-months to five-years-old, supervised by 14 staff. The children were observed finishing breakfast, playing on the playground, participating in circle time, singing, and rotating through structured centers. Six child records and five staff records were reviewed. The Center has 15 staff with current certification in CPR and First Aid, and 15 staff trained in Daily Health Observation. Four medications and authorization forms were reviewed, and the center has six staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, allergy list, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 10/31/23 and the most recent Health Inspection on file was dated 12/13/23. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 5/17/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.
Based on observation and review of documentation, the center did not ensure that for each group of children, a written record of daily attendance was maintained that documented the arrival and departure of each child in care as it occurred. Evidence: At approximately 9:10am on 5/8/24, there were 13 children observed in the Two?s classroom. The attendance contained documentation of 11 children present. The assistant director added a child to the attendance who had recently arrived. The other child who was missing from the attendance record had transitioned from another classroom at approximately 8:10am.
Plan of Correction:
We will ensure all staff is documenting each child's arrival and departure. Management will be conducting attendance sheet checks daily to ensure documentation is accurate. The staff member was retrained on attendance policies on 5/9/24.
