Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring/renewal inspection was initiated on 03/10/2021 and concluded on 03/15/2021. The provider was contacted by email to initiate the inspection. There were 1 children present and 1 caregiver. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 caregiver record submitted by the provider to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the family day home. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based upon review of two children's records, the provider has not ensured that each child's record contains the names, addresses and telephone numbers of two designated persons to contact in case of an emergency if the parent cannot be reached. Evidence: 1. The record provided for child 1 contained the name, address and telephone number of only 1 designated person to contact in case of an emergency if a parent cannot be reached. 2. The record provided for child 2 contained the name, address and telephone number of only 1 designated person to contact in case of an emergency if a parent cannot be reached.
Plan of Correction:
The provider responded with the following: Work contact information is on file for both families. The parents will be required to provide information regarding a second emergency contact person.
