Technical Assistance:
A resource guide was emailed to assist with obtaining all required background clearances. As discussed, all adult household members are required to have a fingerprint, central registry and a sworn statement clearance on file.
Discussed the potential of withdrawing the Voluntary Registration application and re-applying at a later time. Provider will inform the Licensing Inspector of the decision to do so prior to their expiration on 07/20/2024.
Comments:
A renewal inspection of a Voluntary Registered Family Day Home (VR-FDH) was conducted by licensing staff, between the hours of 10 am to 12 pm. At the time of inspection, there were two children present b(ages 9 and 11 years) belonging to the provider. The inspector reviewed compliance in the areas found on the Health and Safety Checklist and background checks. Since there is currently no enrollment, children records were not reviewed. The 1 household record was reviewed. Two additional household members who were not listed on the renewal application were not present during the inspection. Background clearances for the two additional household members were not completed.
Information gathered during the inspection determined that there were was a non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the home.

Based on statements from the provider, a Sworn Statement was not provided for 2 household members. Evidence: Household Member #1 and #2 has not completed a Sworn Statement or Affirmation.

Based on review of background clearances, copies of the results of Central Registry searches were not maintained. Evidence: 1. A Central Registry clearance was not maintained for the Provider and Household Member #1 and #2.
Plan of Correction:
All required background searches will be requested and the results will be maintained. The results will also be forwarded to the Licensing Inspector for review.
