Comments:
An unannounced renewal inspection was conducted beginning at 12:00 p.m. and ending at 1:15 p.m. There are no children currently enrolled or in care and not all requirements could be reviewed. Documentation was reviewed including the provider record.
Information gathered during the inspection determined non-compliance with applicable standards or requirements and violations were documented on the violation notice issued to the program.

Based on review of documentation and interview with the provider, it was determined that the provider failed to notify the organization within 14 calendar days of any identifying changes. Evidence: The provider moved homes in September of 2021 and did not report the change until October of 2022.
