Comments:
An unannounced monitoring inspection was conducted from 11:45 a.m. - 1:45 p.m. There were 44 children in care being supervised by 10 staff members. The children were observed participating in structured center play, circle time, eating lunch, and preparing for nap. Lunch served today is turkey and cheese on tortilia, salad with ranch, mixed fruit and milk. Ten children's records were reviewed and in compliance. Five staff records were reviewed. Four medications were reviewed. There are currently three staff members on site who are MAT (Medication Administration Training) certified. Based on observation of the attendance sheet in the two year old classroom it was determined that a written record of attendance was not kept daily. Evidence: The attendance sheet for the two year old classroom was not complete for today, 08/22/2019.

The staff will update the attendance sheet for today.

Description:
Based on observation of four medications it was determined that not all children with prescription medication had documentation of a parental authorization form. Evidence: The emergency medication observed for child A did not have an authorization form signed by the parent and physician.

Administration will request an authorization form to be completed by the parent and physician for child A.

Based on observation of the posted inspection it was determined that the findings of the most recent inspection was not posted. Evidence: The most recent inspection of the facility was conducted on 06/14/2019. The inspection posted today is dated 01/15/2019.
