An unannounced Subsidy Health and Safety self report was conducted from 10:45 a.m. - 11:30 a.m. regarding notification requirements. Interviews were conducted, documentation and recorded video footage was reviewed. The self reported incident regarding notification requirements was determined to be valid.
Based on interviews and documentation review it was determined that not all injuries that required treatment from a medical professional were reported to the department within two business days. Evidence: On 01/15/2020 child A (Age 4) sustained an injury at the facility which required medical attention. It was later reported by the parent to the facility on 01/15/2020 at 5:46 p.m. that child A fractured their their upper left arm. The facility notified the department of the injury on 01/23/2020.
Plan of Correction:
Management at Nanda Learning Center will inform VA Licensing of any injury involving the need for medical care within 24 hours of notification from parent or on premises of center following the injury.
