Comments:
An unannounced monitoring inspection was conducted from 10:45 am - 1:15 pm. There were 36 children in care being supervised by 7 staff. The children were observed participating in outdoor water play and circle time activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of four staff records it was determined that not all staff records contained documentation of a TB (tuberculosis) screening prior to coming into contact with children. Evidence: The record for staff A (start date: 2/8/23) did not contain documentation of a TB screening.
Plan of Correction:
Staff A will obtain a TB screening and submit the results to administration for their record.

Description:
Based on observation it was determined that not all procedures for diapering were followed. Evidence: 1.) The foot operated mechanism on the trash can in the infant room and the two year old room was not functional. 2.) There is a rip on the bottom right corner of the changing pad in the toddler room making the changing pad absorbent.
Plan of Correction:
1.) Administration replaced the foot operated trash cans during the inspection. 2.) Administration has already ordered a new changing pad.

Description:
Based on review of medication it was determined that not all procedures for administering medication in regards to ensuring methods to prevent use of outdated medication were followed. Evidence: The emergency medication on site for child A had an expiration date of 03/25/2023.
Plan of Correction:
Administration will contact the parents of child A to obtain a replacement medication that is up to date.
