Technical Assistance:
Children under age 10 shall always be within sight and sound supervision of staff. Additionally, staff are required to follow playground procedures to ensure the provisions of active supervision by staff to include positioning of staff in strategic locations, scanning play activities, and circulating among children.

Comments:
An investigation was initiated from 3:15 pm - 4:30 pm to investigate a complaint regarding supervision, administration, and procedures for emergencies. Interviews were conducted, documentation, child records, and staff records were reviewed. The complaint regarding supervision, administration, and procedures for emergencies is determined to be valid.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Complaint related: Yes
Description: Based on interviews it was determined that the center failed to inform the department's representative within two business days when an injury to a child occurs while under the supervision of the center that requires outside medical attention. Evidence: On 6/15/2023 child A (age 15 months) sustained a burn to the inside portion of their left thigh. Child A was taken to urgent care by their parents for treatment. This incident was not reported to the office of child care health and safety as required.

Plan of Correction: On Thursday, July 6th, 2023, the director and owner held a meeting with our staff members to review the center's privacy policy pertaining to its parents and their children, and to emphasize the importance of supervision. In addition, we will hold a training workshop in October to discuss health and safety in the workplace and provide information to further staff members' understanding of the importance of supervision.

At Nanda, safety is out top priority. We also strongly believe in transparency, which helps us to build a strong trusting and supportive relationship with our community. We are committed to upholding our excellent standards and practices.

Description: Based on interviews it was determined that staff did not always ensure the care, protection, and guidance of children under their supervision. Evidence: On 06/15/2023 staff could not explain how child A (age 15 months) sustained an injury to the inside portion of their left thigh which was later determined by medical staff to be a burn. Staff observed the injury around 11:20 am during a diaper change after children returned to the infant classroom from outdoor play. Staff reported that they initially thought the injury was a bug bite, which was reported to the parent of child A via a phone call. The facility requested that the parent pick up the child from the facility. The parent subsequently took the child to an urgent care facility for diagnosis and treatment where the injury was determined to be a burn. The parent later inquired with administration as to how their child sustained the burn. Administration could not determine, but could only speculate that the injury could have potentially occurred on a hot surface on the playground such as a slide, pour in place surface, or from a tarp that the children were sitting on for a group activity. The reported temperature on 06/15/2023 was mostly sunny with a high of 83 degrees.

Description: Based on interview it was determined that the center failed to maintain a written record of children's serious injuries on the day of occurrence. Evidence: On 06/15/2023 Child A (age 15 months) sustained an injury to the inside portion of their left thigh that was later determined by medical staff to be a burn. The facility reported the injury to the parent over the telephone however a written injury report was not created to document the incident.
