Comments:
A self reported investigation concerning supervision was initiated on 07/01/2024 from a report received by the Fairfax County Licensing Office on 06/24/2024. The investigation was conducted on 07/01/2024 between 10:09am to 10:57am, and completed on 07/02/2024. A tour of the facility, video footage, staff statements, 2 staff files and 1 child file were reviewed. Based on the preponderance of evidence gathered, violations were cited.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on observation of the center's video recordings and staff interviews, staff did not ensure care, protection and guidance while supervising children.

Evidence:
1. On 06/24/2024, at approximately 12:01pm, Staff #1 and Staff #2 left child #1 (13 months old) on the playground located across the interior road from the school while transitioning their group of students back to the center.

2. Staff #1 and Staff #2 confirmed they failed to perform a name-to-face check before exiting the playground, resulting in their unawareness that Child #1 had been left behind.

3.After crossing the interior road and entering the center's front door to conduct a headcount, Staff #2 noticed child #1 was missing and immediately returned to the playground to retrieve the child.

4. Based on video footage, Staff #2 found Child #1 on the enclosed playground at approximately 12:02pm. Child #1 was left alone and unsupervised on the playground for approximately one minute.

Plan of Correction: Staff training on Name-to-Face, threshold counts, and supervision of children was completed or reviewed by all staff members.

Staff training on federal, state or local licensing requirements in addition to Goddard policies and procedures on children supervision was completed.

Name-to-Face and Threshold counts signs were posted outside on the playground as a reminder to all staff members.

Standard #: 8VAC20-780-340-F
Description: Based on observation of the center's video recordings and staff interviews, the center did not ensure that all children under 10 years of age were always within actual sight and sound supervision of staff.

Evidence:
1. The center's video footage documents that on 06/24/2024 at approximately 12:01pm, Child #1 (13 months old) was left behind on the enclosed playground while Staff#1 and Staff #2 transitioned their group of student back to the center.

2. At approximately 12:02pm, Child #1 was found alone and unsupervised by Staff #2.
Child #1 was not within actual sight and sound supervision by Staff #1 and Staff #2 for approximately 1 minute.

3. Staff #1 stated that during the transition from the playground to the classroom, Staff # 1 was rushing to return to the center and placed 1 of 5 children in the stroller. Staff #2 turned back to get a mat on the playground and Staff #1 assumed Staff #2 was getting Child #1.

4. Staff # 2 stated that when leaving the playground, Staff #2 placed 3 of 5 children in the stroller and assumed Staff #1 had child #1 in the other stroller.

5. Staff # 1 and Staff #2 both stated that they did not do a name-to-face headcount of the children before exiting the playground.

Plan of Correction: Staff training on Name-to-Face, threshold counts, and supervision of children was completed or reviewed by all staff members.

Staff training on federal state or local licensing requirements in addition to Goddard policies and procedures on children supervision was completed.

Employee handbook was reviewed by staff on failure of not complying to the federal, state or local licensing requirements and Goddard policies will result in disciplinary action and or termination of employment.
