Comments:
An unannounced monitoring inspection was conducted today from approximately 3:15pm-5:00pm. There were19 children (3yrs-5yrs.) directly supervised by 4 staff. The physical plant, 3 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books,working in letters with the teachers. There was an adequate supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Based on review of child records it was determined that all required information was not maintained.
Evidence: During inspection Child 1 and Child 4 were missing the work phone number for 1 parent listed
