Comments:
An unannounced monitoring inspection was conducted today from 12:40 pm -2:15 pm. There were 12 children ( 39 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping and arriving into care. Diapering procedures were also discussed. There was an ample supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Based upon records reviewed, required information was found to be incomplete.
Evidence: Child #1's record is missing emergency contact addresses. Child #2's record is missing authorization for emergency medical treatment. Child #3's record is missing emergency contact ( name, address, telephone numbers) authorization for emergency medical treatment.

Description:
Based upon observation, caregiver exceeded 16 points upon inspector arrival.
Evidence: The inspector arrived into the program at 12:40 pm at the same time Caregiver 1 was arriving back to program with 1 child ( 3 points) Caregiver 2 was inside with sleeping children ( 29 points)
Plan of Correction:
I do not usually leave the program. The school is right next to my home and there was an emergency today and the child's parent asked me to pick them up. This is the first time this has happened and I will not leave again. I was gone less than 5 minutes. The child usually comes after school and 2 other children go home by then so I am always in points..I will not do it again.
