Comments:
An unannounced monitoring inspection was conducted from 9:15am-11:20am. At the time of entrance, 20 children were in care with 4 staff members present. Children were observed during snack time, playing with puzzles, reading books, dancing to music, engaged in group activities and coloring. The site was clean and organized. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Based on review of five children's records, the center did not maintain complete information. Evidence: 1. Child #2's (start date 9/04/2019) file was missing documentation of proof of the child's identity and age.. 2. Child #3's (start date 9/03/2019) file was missing the child's two emergency contact addresses. 3. Child #5's (start date 9/30/2019) file was missing the child's two emergency contact addresses.

Description:
Based on record review, the center did not have one staff member on duty that has obtained within the last three years instruction in performing the daily health observation of children. Evidence: 1. The four staff records reviewed did not have documentation of daily health training in the last three years.

I will look for the documentation and send to you and if I am not able to find it, we will retake it.

Description:
Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. A can of Lysol was on top of the classroom counter and was not kept locked.
Plan of Correction:
The cleaning supplies were moved during the inspection.
