Technical Assistance:
Discussed training hours and the requirement to complete 16 hours per year. Discussed the need to do two shelter in place drills each year. Discussed sight and sound supervision of children. Discussed volunteers are not counted in staff to children ratios. Discussed cleaning supplies must be kept locked when not in use. Discussed children's records and the requirement to have two emergency contacts that can not be the child's parents.
Comments:
An unannounced monitoring inspection was conducted on 3/02/2022. At the time of entrance, 14 children were in care with 1 staff member present in the classroom. The classroom was over ratio during the inspection. A volunteer was present in the classroom with children, but did not count in staff to child ratios. The 2nd classroom did not have children present and had 1 staff member present. The children were observed working on handwriting skills. The site was clean and contained a supply of developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. There were no medications on site. The center did not have a person on site or available with MAT training. Areas of non-compliance are identified in the violation notice. Based on review of children's records, the center did not have immunization documentation for a child prior to the child's first attendance. Evidence: 1. Child #2 (start date 1/15/2022) did not have documentation that the child has received the immunizations required by the State Board of Health.
Plan of Correction:
Immunization records will be reviced for all students and asked to be updated by no later than March 25th f 2022

Description:
Based on record review, children's records did not have complete information. Evidence: 1.Child #1 (start date 10/18/2021) did not have one parent's place of employment and Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. 2. Child #2 (start date 1/15/2022) did not have one parent's place of employment and work phone number for both parents, Name, address, and phone number of one designated people to call in an emergency if a parent cannot be reached. and documentation of health information as required by 8VAC20-780-130.
Plan of Correction:
Adjustments will be done to the system to require parents to fill all the information or the system will not accept their registration Information of those students already registered will be updated by March 31st of 2022

Description:
Based on observation, hazardous substances were not kept in a locked place. Evidence: 1. Seven cleaning supplies on the top of a cabinet in the 2nd classroom were not kept locked.

Description:
Based on observation, the center did not follow staff to children ratios when children were in care. Evidence: 1. During the inspection from approximately 10:26am to approximately 11:27am, the center had 14 children with one teacher in the 1st classroom (Ages 2 years old through 5 years old). The center had a volunteer present in the classroom that was not counted in the staff to children ratios.
Plan of Correction:
There are always 2 teachers per classroom for a total ratio of 7:1 plus tehe Director. The director was not on site but we had a volunteer with us. The teacher had to step out of the classroom to find the information needed for the inspection. Next time an inspection is pre-announced, we will make sure the folders and possible information needed is moved to the classroom so the teacher will not have to step out.

Based on observation, the number of the regional poison control center was not posted in a visible place. Evidence: 1. The center posted 911, but did not have the regional poison control center posted in a visible place.
Plan of Correction:
The regional poison control number has been already placed in a visible site inside the classroom and in the office (Classroom 2)
