Comments:
An unannounced monitoring inspection was conducted on 2/12/2024 from 9:22am to 10:18am. At the time of entrance, 5 children were in care with 2 staff members present. Children were observed participating in a group activity and coloring a worksheet. The children were speaking and learning the Spanish language. A selection of staff and children records, the physical space, evacuation drills, first aid kit, emergency plans and attendance records were reviewed. The site was clean and organized. Interactions between the children and staff were positive. Areas of noncompliance are identified in the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Repeat Violation Based on record review, one of four children's records did not have complete information. Evidence: 1. Child #2 (start date 9/05/2023) did not have both parent's work phone numbers, one parents home address and two emergency contact addresses.
