One area of non-compliance of the supplemental subsidy health and safety requirements was found. Questions about this inspection may be directed to pamela.sneed@dss.virginia.gov

Violations:
Standard #: 22VAC40-665-170-B-12
Description: Based on records reviewed and interview conducted, it was determined that 1 of 2 children's records did not include a written allergy care plan written by a physician.

Evidence:

1. The record for Child #3, included documents signed by the physician in August 2020, that documents the child has multiple food allergies.

2. The same record, included enrollment paperwork signed by the parents in November 2019, that documents the child has multiple food allergies, and treatment requires over-the-counter medication followed by a prescription injection.

3. The provider said that the parents had mentioned the child having food allergies, though she thought no serious, nor did she know what actions, if any, she needed to take if there was an allergic reaction.

Plan of Correction: The provider will request parents to provide a written allergy action plan from the physician, or provide documentation that the child no longer has allergies that may require medical actions.

If the allergy action plan obtained requires medication administration, the provider will obtain the required training to administer the medication to the child.
