Technical Assistance:
1. Discussed the safety concerns and removal of a mesh-side fabric tied onto the crib today. The provider explained that it was a request of the parent, but stated she would remove it.

Comments:
An unannounced monitoring inspection was conducted at 10:15am. Observed 7 children = 18 points with the Provider. Ratios were not maintained, though supervision was in compliance. Children were playing with toys, reading books and having story time, dancing to child appropriate music videos, and taking a water break. Children participated in cleaning up toys after each activity. Each child brings their own food daily. The daycare areas of the home were found to be clean and sufficiently supplied with toys and equipment for the children. The provider was actively engaged with the children and leading activities. Standard #: 8VAC20-800-90-C
Description: Based on records reviewed, it was determined that 2 of 4 children?s records reviewed did not include immunizations updated every 6 months until the age of 2 years old.

Evidence:

1. Child #13 ? The immunizations were dated December 2020, when the child was 16 months old.
2. Child #14 ? The immunizations were dated December 2021, when the child was 16 months old.

Plan of Correction: The provider stated that she has already requested the immunizations from the parents, but has not received them yet. She agreed to request again.

Standard #: 8VAC20-800-570-A
Description: Based on observations made and records reviewed it was determined that the staff:child points exceeded 16 points per caregiver. Evidence: The provider was working alone today and caring for 7 children = 18 points.

Plan of Correction: The provider stated that she was unaware of being over in points and was unclear in the age-to-points breakdown. After the discussion with the inspector the provider indicated that she now understands how to calculate the points. One child will have a birthday in 2 weeks that will drop the points, and a 2nd child has a few weeks afterward that will drop the points again.
