Comments:
An unannounced renewal inspection was conducted on 02/13/2023 from approximately 10:54 a.m. ? 1:37 p.m. Caregiver, household members, children?s records and the physical plant were reviewed. There were 7 children present that were between four years old and seven years old with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of 5 child records were reviewed. Caregiver and household member records were reviewed. The information gathered during the inspection determined violations with applicable standards or law. A violation notice was issued. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 02/20/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Based on children's record review, the provider failed to ensure that one of five records contain documentation of the review of parental emergency contact information required by 8VAC20-800-60.B.2 and parental signed acknowledgement of the review.
Evidence: The record for C3 enrolled 01/04/2022 failed to contain a parental signature acknowledging the annual review of the child's parental emergency contact information.

Description:
Based on observation and interview, provider failed to assure that food containers brought from home shall be clearly labeled in a way that identifies the owner.
Evidence: Licensing inspector observed children eating their lunch during the inspection. Food containers brought from home were observed not being labeled to identify the owner. Provider advised she has requested parents to label food containers without regular compliance.
