Comments:
An unannounced monitoring inspection was conducted on 11/08/2023 from approximately 11:40 a.m. ? 1:15 p.m. Caregiver, household members, children?s records and the physical plant were reviewed. There were 7 children present that were four years old with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of 5 child records were reviewed. Caregiver and household member records were reviewed. The information gathered during the inspection determined violations with applicable standards or law. A violation notice was issued. Please sign the violation notice and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Based on children?s record review, the provider failed to document verification of the identity and age of the child within seven business days of the child?s first day of attendance at the family day home. Evidence: The records for C1, C2, C3, C4, and C5 did not document the date the provider verified the identity and age of the children. The element on the form was blank.

Description:
Based on child record review, the provider failed to obtain documentation that each child was adequately immunized accordingly to the requirements of the Code of Virginia and applicable State Board of Health regulations prior to attending the family day home. Evidence: Immunizations for C4, enrolled 07/08/2023 were dated 08/11/2023. Immunizations for C5, enrolled 08/07/2023 were dated 08/10/2023.
